Lumbrosacral region swelling..

6 months old male infant presented with a swelling in lumbrosacral region since birth which is gradually increasing in size. The swelling gets aggravated when the baby strains during coughing and crying .Mother says that the child could not move both lower limbs properly .No maternal disease during pregnancy .No history of used drug during pregnancy . Full normal delivery and normal APGAR. On examination there is soft cystic swelling in the midline over the lumbrosacral region. Fluctuation is positive .Transillumination is positive and during transillumination fine strands are seen within the swelling. The swelling is partially compressible .Both the lower limbs are flaccid and sensation of lower limbs appears to be less that the upper limbs and there is bilateral club foot. A bony gap can be felt at the margin of the swelling. The fontanels are normal and the head circumference is ………
