A 65 years old man came to out patient department complaining of abdominal distension and pain at right hypochondriac region . On examination spleen was palpable 2 cm below the left costal margin , skin was flushed , tachycardia and low blood pressure (90/60). After further examination he was diagnosed as liver cirrhosis but the cause was not found.
In this patient what is the cause behind tachycardia and low blood pressure?

Generally liver cirrhosis is associated with hyperdynamic circulation and peripheral vasodilatation ,So there is Na+ H2o retention. Due to the consequent activation of the sympathetic nervous, renin-angiotensin-aldosterone and vasopressin systems.
so there is tachycardia and low blood pressure..
Rt Hypochondriac pain as mentioned there refers to Congested Liver likely secondary to heart failure as a result of the heart unable to maintain tissue oxygenation due to persistent third spacing of the intravascular volume.So probably heart failure expalins hypotension and tachycardia..
